DeleteCustom forms aren't as scary as they look. I used to get super nervous but really it's just "copy addresses, write what's in box, check box that says this thing is not going to damage anything else, done" also write both addresses all over the box just in case but I do that anyway. The true challenge is when the customs forms (to affix to package) are bigger than the package itself.
